I did. I was looking through kotaku.com, and found this link. Hopefully this happens to all Wii GHIII owners who sent in their discs!

Redoctane's site. You can find all the needed info there. Unless you have surround sound, it's pretty useless to send it in.

But you get a free faceplate so its worth it

It is at the activision website. I got my replacement disk about 1-2 weeks ago, but I never received a faceplate... just an envelope with the remastered disk. =\ I'm checking into this. I want my faceplate! lol.
